Lost or Stolen Passports

The Embassy accepts applications to replace passports that have been lost, stolen, or mutilated (damaged).

An applicant without a passport, due to loss or theft, must come to the Embassy and:

  1. Appear in person and sign the passport application, form DS-11, before a consular officer.
  2. Complete and sign the:
    Statement Regarding Lost Or Stolen Passport (DS-64).
  3. Bring (if you can) some form of identification (e.g. driver’s license, residence card) as well as documentation that establishes your citizenship (e.g. birth or naturalization certificate, an expired passport).
  4. Bring two identical photo (5cm x 5cm on a white background)
  5. Pay the appropriate fee in US Dollar cash ($100 USD).

Replacement passports can be issued within two to three weeks.  In cases where the applicant must travel urgently, the Embassy can issue an 'emergency passport' which is valid for one year. This passport is valid for all travel. This 'emergency passport' can be exchanged later for a full validity passport at an embassy or consulate overseas or at a Passport Agency in the United States.

Report a lost/stolen passport to the police

A U.S. passport is a valuable document, not just for you the rightful bearer.  Therefore always report the loss or theft of a U.S. passport to the local police authorities to avoid Identity theft!  How can someone steal your identity? Identity theft occurs when someone uses your personal information such as your name, Social Security number, credit card number or other identifying information, without your permission to commit fraud or other crimes.
